I use two companies:
BatchPCB -> They are run by SparkFun Electronics. Boards are good quality. Spark fun publishes the parameters for DRC, plus you can download Eagle CAM files for Gerber generation. You usually have to wait a little longer to get them, but they are cheap. The one good thing I have noticed is they usually ship you more boards then you ordered. The last board I sent them was my own design of a 16 channel DMX controller measuring 3.75 x 8 inches, the price was $90 for one, which was expensive, but cheaper then other companies. THEY SENT ME THREE!!! so final cost was about $30 a piece.

Gold Pheonix -> This is the house that BatchPCB uses. They have a special were you purchase 155 sq inches of PCB and however many of your designs will mathematically fit into that space is how many they send you. I have placed my first order with them for the 16CH DMX board listed above and am waiting for them to return. Since BatchPCB uses them, it should be equivalent quality, I will know when I get them I guess. They accept Gerber files and you can use the DRC parameters published for BatchPCB. One note, you have to jump through a few hoops to order as they do not have a way to submit online, all has to be done by e-mail. I followed their instructions and it seemed to go smoothly. BTW: With them, my 3.75 x 8 board came out to be $110 for 5, or $22 a board which is not bad for small quantities of a board of this size.
